Consultations and Services

Initial Consultation - FREE (1 hour)

Meeting with the client and primary caregiver to identify needs. The goal is to not only meet the basic care needs but to discuss planning to prevent caregiver burnout and long-term success. This includes an assessment of the current level of care and support system and goal setting for the future. We will review your current insurance and available resources through your PCP and the state of Michigan. We will have a high-level discussion regarding connection options and next steps.

Home Management and Comfort Coordination - $20/hour, 1 hour minimum

Upon completion of the home safety assessment, Community Connections can help guide you on the path of success by creating a home management plan. This includes:

  • Schedules of household duties, communication logs, and organization of companion or referral services in a weekly or daily calendar

  • Organization of high-risk areas in your home

  • Coordinate safety measures such as grab bars, ramps and trip hazards

  • Provide home layout recommendations

  • Routine simplification

  • Advance Directive Planning

Monthly coordination of services including scheduling companion support, transportation, medical appointment companion if needed. Service also includes cancellation coordination and rescheduling. Includes household management services end of life or termination of services communication

Basic Referral Connections - $10 per referral

A home safety assessment will be conducted, and I will provide a personalized connection plan based on our initial consultation and home safety assessment. This will include available services and next steps along with basic referral options.

Comprehensive Referral Connections - $20/hour, 30 minute minimum

Community Connections will connect resources with the client based on the client’s needs and companion’s expertise with no appointments needed. Service includes scheduling of one-time resources such as companion or transportation introductions, scheduling donation pick up, etc. The client will be responsible for rescheduling if needed and any subsequent scheduling. This service can be provided as part of any previous appointment if time allows at no additional cost.

Monthly Home Management - $20/hour, 2 hour minimum

Onboarding and Recommendations - $50 (1 hour)

If you would prefer to not have an appointment or consultation, Community Connections can still provide a list of trusted resources for your needs at that point in time. All referral sources have been screened by Community Connections and meet our Quality and Community standards of excellence. Basic Referral Connection services will require the client to make phone calls, schedule services, etc.
